They then navigate and facilitate care between other service providers, acting as the client’s advocate. LSK Case management is client-focused, centered on individual client needs, with the client’s best interests driving all coordination and care. Our Case Managers often help the client secure resources to learn the day-to-day skills necessary to live successfully in recovery within their own unique family system and community.
